Request a Free Poison Safety Packet

Marylanders and D.C. residents in the Maryland Poison Center's service area are invited to request a poison safety packet for their home.

This free packet includes poison safety information, Mr. Yuk stickers, telephone stickers, and a magnet. To request your packet, please complete and submit the form below.

Once your form has been submitted, you should receive your packet at the address that you provided in 10-14 days.

Please Note: Individuals who indicate that they reside outside of Maryland or Washington, D.C. will be redirected to the Americas Poison Centers website, where they can locate the poison center that serves their state.
