Request a Free Poison Safety Packet

Marylanders in the Maryland Poison Center's service area are invited to request a poison safety packet for their home.

This free packet includes poison safety information, Mr. Yuk stickers, telephone stickers, and a magnet. To request your packet, please complete and submit the form below.

Any material orders placed on or after 12/18 will not be filled and mailed until 1/5.

Once your form has been submitted, you should receive your packet at the address that you provided in 10-14 days.

Please Note: Individuals who indicate that they reside outside of Maryland will be redirected to the Americas Poison Centers website, where they can locate the poison center that serves their state.
